FOGS Requirements, Reports and Forms

Food Service Establishment Questionnaire must be filled out by all Food Service Establishments as part of the Business License approval process.

Grease Trap & Interceptor Maintenance Report must be filled out by Food Service Establishments as a quarterly report.

The above reports must be submitted to Payson City Pretreatment Program by fax at 801-465-5280 or call the Pretreatment Coordinator at 801-404-6476.
